Writing good web app documentation

Does anyone have any advise on writing documentation for web apps they have created for users?

Are there any recommended formats for this type of documentation? Headings, layout etc?

Anything at all, from small little apps that people can download and install themselves to larger SAAS apps.

Try to find examples of other apps that have clear and concise documentation

Some of the open source projects have wonderful technical documentation, but the actual user docs can be woeful. The one exception that springs to mind is OpenX (formerly phpAdsNew)
